how do I publish a form so others can view fields

I have designed a new message form with many needed fields but when I send
this new form to anyone, they cannot see the fields (they only see a regular
mail message not my newly designed one). I've tried publishing it, etc. to
no avail. Help!!! I am not familiar with Outlook at all!
 
What precisely are you sending to other people -- a message created with your form or a saved .oft form template file? Did you click the Edit Read Page button in design mode and create a read layout with the desired controls?
