How do I make a vote in an email message?

I am trying to put numerous items in the body of an email to be voted on and
I can only figure out how to just vote yes/no to an email.
 
Outlook voting buttons allow only one question per message. Your other options for conducting surveys depend on your Office version, mail environment, etc. Outlook custom forms generally are not a solution.
