how do I make a document into a template

I am trying to make a document into a template. I followed the instructions
and saved it as a template, but I can't get it to work. I want to creat this
form with lines that can be typed over and not have them move.

You don't just need to make a template, you need to make a specific type of
template for a form. See

I'd guess you'd want to create a document as a form instead. Look for the
help results for CREATING A PRINTED FORM.
