how do i insert tick boxes on word document?

I am currently creating a questionnaire to use as part of a research project.
I am going to email the questionnaire out and need some way for participants
to indicate their responses so they can then email me back their completed
questionnaires. Im not great with computers so any advice will be
appreciated!
 
You'll want to create a form. Go to view-->toolbars-->control toolbox
From this toolbar, you can insert checkboxes, textboxes etc.
Mouse over the icons and it will tell you what they are.
