How do I fill a scanned form in Word?

I downloaded a Dental form which I will use numerous times. I would like to
fill in the blanks, keep the completed form on file, and bring it up to print
whenever I need it. I am unable to fill out the form. What do I need to do to
accomplish this task??
 
Method 1: superimpose textboxes where the data is to be inserted.

Method 2: start again. Create your own form that replicates the one you
downloaded.


Method 1 is less work up front, but more effort each time you use the form.
Method 2 is justified if you really do mean numerous. The theoretical method
3 -- convert the scanned form to text, then convert the text to a form -- in
practice is (almost) always more work than method 2.
 
If 'numerous' means a lot and not merely two or three, and you will be
filling it with different data, then look out for a remaindered copy of
Abbyy Finereader 6 Corporate version, which included a brilliant form
filling tool. I believe this is now available as a separate programme, but
whether it remains as easy to use I cannot say.

Otherwise Jezebel has the Word answers (though personally I would use an
unbordered table rather than text boxes to layout a grid over the background
image of the form - and I would use form fields in the cells to accept the
data).
