how do i create Tick Boxes in word?

First select the 'Forms'-toolbar. (Right-click any toolbar and select).
Then insert the tick-box that is on the toolbar. To be able to tick it you
have to protect the document as a form (or just the section in which you
have the box). Do that bij selecting Protect Document from the Tools menu.
(I am using a Dutch version so I may have some of the titles wrong...)
