How do I Create interactive questionaires in word ?

I would like to create interactive questionaires using word so that Fencers
at our club can brush up on health a safety can this be done?

Is there a down load that i can obtain i am using M/SXP with word 2000
 
What you are describing, I think, is an online form. See

Word isn't actually designed to create interactive things. So this might be
a bit tricky.

There may be a template available that you could modify for your needs. See
http://office.microsoft.com/en-us/templates/default.aspx to search for an
appropriate template, or search Google for a template.

Alternatively, you can create a Word form. See the following article, and
the links from that article to Dian Chapman's pieces on creating forms.
