How do I create a heading for Microsoft Outlook?

I suggest you do that at the BOTTOM - like every one else does. Have you
looked at setting up a custom template?

You could also try making your own stationary. I would make it in .html
format as you like it. Then in outlook go to the tools\options\Mail
Format\Stationary picker then selcet new. Choose start with a blank
stationary then enter a new name for your stationary and hit next. Choose
picture then browse. Change the file type to *.* and go to where you saved
the html file.
You may have sizing issues, and it may just be best to use the footer
instead of creating a header.
