How do I create a Fax Cover Sheet from a contact?

I would like to be able to create a Fax Cover sheet as you can a letter
directly from my contact list. I know it can be done, but cannot figure out
how. Can anyone help?
 
Most people use a mail merge, then just save the merge document for reuse.
