Is there anyway to get the hierarchical start menu back? I can't seem
to find the option. Is it gone for good?

Do you really mean Hierarchial or Cascading? Right click Taskbar > Start
menu (tab) > select the "Start menu" radio box, then click OK.

Cascading the way it used to be in WinXP, but I don't want the Classic
Menu look either.

Skipped a step. It's Taskbar>Properties, etc etc etc.

At least that's the way it is in Ultimate 64-bit.

No Cascading menus are not a part of the two pane start menu in Vista. I
think they should have made it an option though.

I was puzzled by this as well. The Classic Start Menu option
specifically says
it looks like 'previous versions of Windows.'

Apparently XP was not a previous version of Windows. Who knew?

The Windows XP Luna theme was dropped from Vista. The only themes in Vista
are Windows Classic, Windows AERO Glass, AERO Basic, and a sort of AERO
Express if you are running Home Basic. The two start menus included are
Classic and the new Hierarchial tree menu.
