header/footer toolbar is missing, how can I reinstall

Since, the header and footer is seldom referred after
initial creation, it is not parked permanently on the Tool
bar. Choose Header and Footer from the View drop down menu,
the tool bar appears only during usage.

If you work primarily in the header and footer, right click
on an empty space on the tool bar, choose Customize. In the
dialog box that opens, click on the Toolbars tab. Check or
uncheck the items listed.
 
I have tried all that, thanks, but the header/footer toolbar is not listed. I
think another user has tampered with my settings, so now I wish to reinstall
the toolbar.

hope you can tell me the file name.

I eventually found the toolbar, it is hid with the others at the top of the
screen, how can I make it 'float' again, like before?

Every toolbar (and the menu bar, too) has a "handle" at the left end. In the
older versions it's two light-gray vertical stripes, and in the newer
versions it's a vertical row of gray dots. When the mouse hovers over the
handle, the mouse becomes a 4-way arrow. When that appears, click and drag
toward the middle of the screen. Presto, the toolbar floats.

You can also resize a floating toolbar so it's more square, by dragging its
sides or top/bottom.

You can drag any toolbar or the menu bar to any of the four sides of the
screen, and when you drag far enough it will "dock" there. On the left or
right side, the bar turns vertical.

make sure the 'VIEW' header and footer is selected, you can see the header
at the top of the page. place cursor over a vertical line on the right of
the toolbar. it turns into a multi directional arrow. Then just pull your
header and footer toolbar back on to the page.
