Getting Word to create a templete like Word Perfect does

In Word Perfect I can create templetes where as soon as I open the document,
a box pops up and asks for all the information that will be needed to
personalize that document. I will in the box, click ok and there is my
completed document. Is there a way to do this in Word? Any help would be
greatly appreciated. Thanks

Yes, in Word that kind of popup is called a userform. See
