I am trying to create a form in which all one must do to complete it is click the line where the information needs to go. I want it to look like this:

Name: ___________________

And upon clicking that line, the person filling out the form would be able to put in that information. Thanks!
 
What you are talking about is what Word calls an "online form." Check this
