Forms in Word 2003

Does anyone know a way to put a check-box in a form that when you went to
check the box, instead of putting a check mark or X, it would just blacken
the whole box?
 
Take a look at how it is done on the fax template that comes with Word, and
substitute the appropriate symbols for those used in that template.
