You have 4 sets of form tags on the page
- remove the last 3 at the end of page

The reply to field should not have your email address (unless you want to reply to yourself)
If should have the form field name of the use Email address (Email)
| well, rebuiilding and republishing it didn't fix it. I do notice that
| as soon as I close the file on my local hard drive, and re-open it, the
| ability to access the form properties is no longer present. ??
