Formatting and nonprinting characters

Can anyone tell me why a document that has field codes related to a table of
contents would look like this?

If I do a (Shift) and (F9) the field codes do not turn on/off.

There are symbols in the document (specifically the footer) that look like a
thin rectangle. I cannot paste the symbol in this message.

Tables were removed from the document as well where there were simple tables
with single columns.

Any ideas would be appreciated.
 
It looks like you have got a TOC display embedded within your TOC Field. I
would just delete it all and reinsert the TOC.

Shift+F9 only toggles the selected field(s) so won't have an effect unless
it is selected. Alt+F9 toggles all fields, although you may have to hit it
twice if some are showing field codes and some not.
 
It would appear that someone has attempted to create a TOC manually using
hyperlinks and cross-references, but it would appear that the fields are
plain text rather than fields.
