Format changes

When I change the format of selected text, the format of the entire document
is changed. For example, when I change selected text to Bold, the entire
document becomes bold. If I then select Ctrl+z, the rest of the document
returns back and the selected text stays in bold.

This infuriating situation also happens when I add bullets or italics, or
cut and paste text -- sometimes. Sometimes it doesn't happen. I have to
double check the document every time I make an edit.

Why does this happen? How do I prevent it from happening?
