Format a footnote

How can I format a footnote reference to appear underlined followed by a
forward slash. I know who to underline the reference by modifying the
footnote reference style however I can't figure out how to include the
forward slash as a part of the reference number.

I can't imagine the underline looking very good, but you should be able to
modify the instructions here to fit your purposes.

"I want the numbers in my footnotes not to be superscripted, and I want the
numbers to be followed by a dot and a tab"
