Form Letter, Template, Macro?

I have a letter that I send out to often and want to find an easy way to personalize it for each client. There are several parts within the letter that I manually change each time: date, contact's name, contact's address, and a few other fields within the text. Is there a way to take this form letter and be able to fill in information that Word can just plug into the letter? That way I can just type in the particulars and Word can put them in the appropriate fields? I know there's a way to do this, but I've been butting my head up against a wall for a while now
Thank you!

The easiest way is with a UserForm. See
http://word.mvps.org/FAQs/Userforms/CreateAUserForm.htm to get started.

If you are willing to learn programming, the most powerful way is with a
userform, a custom dialog box. However, if you just want to get out some
similar letters from time to time, try an online form. Check this in help.
For more about online forms, follow the links at

Chapman's series of articles.

Hope this helps,
