Form footer date and Page x of x show on each page of form

How do I have the date and Page x of x in the FORM footer show on all pages
of the form? It's a one page form with a lot of records (multiple pages).
 
Again, FORMS are not for printing.

Your REPORT can do this.

What do you mean, it is a one page form with a lot of records (multiple
pages)? How can it be one page and mutliple pages?
