Form Design not available on other computers

I created a form as a second Details Tab on the contact form. (on OL97) Now I have a new computer with OL 2002 on it. I am able to see the contacts from the Public Folder but only with the original tabs - not the one I made. Where is it, is there a way to get it back? I resaved it in a Public Folder but that particular folder does not show up on the folder list on the new computer. Please help ( I am still haveing trouble getting all the custom categories too)
 
Where did you publish (not save) the original form?

In Public Folder/MSA Contacts/Draft
Oh! I found it! NOW how do I make it the default so I don't have to go to File/New/Choose Form.....each time

I will also want instruction on how to have all the contact have the additional form. Right now, some do and some don't. I remember an earlier version updating each contact when you hit Save Form. Now what?
