Hi Ann,
Try this:
1. Go back into Design mode for your form you created.
2. Click on Form, then click on "Separate Read Layout." There should be a
check beside this. Click Yes when a dialog box pops up.
3. Click on the Properties tab.
4. Click the checkbox for "Send form definition with item."
5. Re-publish the form to your Personal Forms Library.
6. Re-send a test message and see if the Read page has your custom items.
One more thing - if you have text boxes that you're filling with data, then
you really need to create a new user-defined field and bind that textbox to
the new field to hold that data permanently - otherwise the textbox fields
will be blank when the recipient gets your email, regardless whether your
form gets through or not...
