Form Confirmation Pages

I need help. I have a questionnaire on my website that my employees fill out
to send me information. Lately, alot of my employees are having trouble
submitting their work. They are not receiving a confirmation page or they
say the site crashes. Most of the time I receive the answers in the form of
an e-mail like I am supposed to, but they still don't ever see the
confirmation page like they are supposed to. Can someone tell me what might
be wrong? Is it my form or the way it is submitted? Any help would be
greatly appreciated.
 
Frontpage server extension, ask your ISP to reinstall them.
