Thanks for your posting here.
Based on my knowledge, this feature is only available on Public Folder. If
you open Properties of your private contacts folder, the "Allow these forms
in this folder" option is grey.
Thus, please confirm you are using Public Folder contacts.

|In the properties of my contacts folder, I want to specify that only a
|certain form be used. Why would this option be disabled? I did not

|any changes (that I know of) in the installation and we are using
Exchange
