extend.dat problems

A user was upgraded to Office 2003. His Outlook starts up and displays the error:

An extension failed to initialize. Can't open file: extend.dat. The file may not exist, blah, blah, blah.

We've tried deleting extend.dat under his profile. We've tried detect and repair. We've tried creating a new Outlook profile. None of this has worked.

I'd really like to repair this without having to reinstall Office. Please, does anybody have an idea on this one?

Start Outlook in safe mode; Start-> Run; "path to outlook.exe" /safe

Disable individual add-ins in; Tools-> Options-> tab Other-> button Advanced
Options-> button Add-In Manager and/or COM Add-Ins

Then rename extend.dat to extend.old and start Outlook normally.

Enable the add-ins one by one (restart Outlook after enabling one). When it
fails to load you'll now what add-in needs to be updated.
