Excell word formula

I want the information that is on the first 4 lines to be automatically
inputted on pages 2-6. Like a header\footer but not.
 
If for printing purposes go to File>Page Setup>Sheet and under "rows to
repeat at top" enter $1:$4 in the refedit box.
