how do i get rid of this EQSURF toolbar? If I go to view,
toolbars and unselect it, it comes right back whether or
not the toolbar is locked.

Try ToolbarCop from http://www.mvps.org/sramesh2k/Toolkit.htm
You should also look for other malware with programs such as Ad-aware and
Spybot
