enable/lock fields within form

After much deliberation I have decided that I want to use the same form to
add/edit data as i use to review the data. I would like a toggle button to
control whether the fields are all locked or whether the fields are all
enabled. Do I need to add the fields to an options group? Any assistance
would be greatly appreciated.
