Embedded formcheckbox on preview

I have a from with many boxes on it. When I preview the form, I don't see the
checkmark boxes. I see the words formcheckbox. I want to see the form as it
actually is. How do I correct that setting? Thanks.

In Tools > Options > Print, under "Include with document", clear the option
"Field codes".
