The following happens sporadically. I send an e-mail message to a
known good e-mail address. Outlook does not report any problem with
the send, nor does my ISP report any problem with the send. However,
the item I supposedly sent does not appear in the sent folder.

Help?!

Is the sent mail stored in an other folder than the sent items folder?
If so, see if the option "Save reply with original message" is enabled.

Tools | Options | E-Mail-Options | Advanced E-Mail-Options

That option means that a reply to a mail will be stored in the same
folder like the original mail - if the original isn't stored in the
inbox folder.

I am glad to know about the store options that are available. However,
that is not the problem apparently. The message I sent was a new
e-mail, and it was received by the person I sent it to, because he
replied to my message shortly after I sent it. I just can't figure
out why substantially all the messages I send are stored in the "sent"
folder, except for the occasional problem.

What version of Outlook?

And what store settings have you established, per Herr Goeller's message?
What version of PST are you using? (Right-click the root, choose
Properties, then click Advanced. Examine the "Format" box and make sure
there's no mention of "97-2002" there; i.e., it says exactly "Personal
Folders File". What I'm getting at is that the older version of PST could
behave the way you're seeing if the Sent Items folder got too full.
