I have a situation that I have no idea how to tackle. I need to figure out
how to build an email request form (not sure if that is the exact term) that
will house a set of qualification criteria with some kind of response form (a
series of boxed options that can be checked or maybe a drop down of choices)
that I can send out to officers for their input, so that I can draw
conclusions based on their answers. Do you have any suggestions?
