There are two ways to accomplish this.
1. Hosting the email and web server yourself
2. Letting another company host the email and web server

You should probably have someone host it for you as it
takes a good decree of technical knowledge to do it
yourself.

HowTo:

1.
First determine that the domain name you want is not taken.
If you want "MyDomain.com" check here:
http://www.betterwhois.com/

2.
Go to http://www.rackspace.com/
This link has a good package. They can customize for you of course

If you properly authenticate to your outgoing server, chances are you can
just set your address to whatever you want on your account properties pages.
That's what I do.

There are also plenty of free email sites, like Hotmail, Yahoo!, Oxymail
(oxymail.org), and "You've Got Post" (youvegotpost.com). Just create a
throw-away address at one of those sites.
