Document Map is scrambled

On some documents the document map is fine. Often a
document seems to become corrupted and tables, figures and
other paragraphs styles appear in the document map. I can
fix the problem each time I open the document by
reapplying styles (Ctrl-A Ctrl-Q). However, when I open
the document next time the problem is still there. Is
there any way to repair a document so that only the
heading styles appear in the document map?
 
Thanks for offering your help. Unfortunately it does not
work.

If I set the outline level to body text, it fixes the
problem while the document is open. However, if I save the
document and reopen it, the problem reappears.

To prevent the problems:
1. Tools > Autocorrect options > Autoformat as you type
tab. Un-tick "Define styles based on your formatting".
2. Use appropriate styles throughout your document and
avoid direct formatting. Word takes more notice of its
built-in heading
styles (Heading 1, Heading 2) than anything else.

To fix problems, find the offending paragraphs. Format >
Paragraph. Set the Outline Level to Body Text (which isn't
the same thing
