disk defrag

My HD is seriously fragmented, but every time I run the
MS disk defrag it just dies out at 13% (jumps immediately
to 100%) and says it couldn't defrag "some" (should read
83% of) files.

How do I make it defrag all/most of my HD?

Your math is in error. 13% + 83% = 96%. What happened to the other
4%?
