Disjointed crossreferences in Word 2002

I joined together a number of individual Word documents into one long
document (3 MB, txt only). I used the "insert file" command in Insert to
combine the individual documents

When I click on the vertical scroller (on the right side), it shows the Page
Number in the document correctly but the accompanying reference to the name
of the chapter bears no resemblance to the actual original document.

These references are seemingly at random.

Is this another mysterious idiosyncrasy of Word 2002? What did I do wrong?
Does it matter?

That last question is a difficult one to answer. FWIW, this is not "normal",
but certainly possible. Sounds like some stuff down in the binary structures
that keep track of things have gotten scrambled. Whether this will cause any
major problems is not possible to predict, but the chance does exist.

You might try saving the document as a web page (the full, round-trip and not
the "filtered" type). Then open that in Word and save again as a Word document
(different file name, of course, so that you don't lose the original). Now
open this and check whether things have settled down at all?
