When I was defraging a computer a message came up stating that not all files
could be defragmented so I viewed the report and it pointed to the
outlook.pst file. What could this mean?

Likely it means that the file was still in use/open. A few fragmented files
are not a problem, so you can just ignore the report.

You could try defrag in Safe mode or after you reboot and before you open
Outlook.

Compacting outlook.pst may also defrag as a compacted file is created. To
compact outlook.pst, in Outlook 2003 go to File, Data File Management.
Select Personnel Folders {the outlook.pst file} and click on Settings. Click
on Compact button. If I recall correctly, Outlook 2002 was the same.
