Database Interface Wizard problems

I have been trying to create a new "Contact Us" form on
our existing website. I am logged in and connected to the
website via FrontPage, and go to File | New, Database
Interface Wizard. When I finish setting up the form fields
and click "Next", I get the following message every
time, "Unable to establish database connection with
database."

I do not know why this is happening. When I created the
same such form on my cloned hard drive copy of the
website, it worked. What am I doing wrong? Am I missing a
program extension to make this work? Please help. Thanks.
 
Are you sure your site is hosted on a Windows server with ASP and Access
support?
