customize a message form

Credits? what do you mean by that?

Why would you want to use a form for something that sounds like it can be
done with a signature?
 
Thank you for responding Sue Mosher. I fell I'm in good hands. What I mean
by credits is a listing of previously published articles. A track record,
bona fides.
I'm going to use a signature as well. Ideally, it would look something like
From: C. Beason
Credits: Hourglass magazine, SDouth Bend Tribune
Subject: Article Query: Harlan County, Kentucky 1944-51
I have 1400 hundred words describing conditions before the social programs.
Could I have the adress for your submissions editor please?

Since email messages don't have anything resembling "headings," I'm at a
loss to understand what you're trying to do.

Subject: Article Query for
 
You mean you want to add a new field to Outlook that every recipient would
be able to see in that position in their email program, regardless of what
it is? That's not possible.
