Customised Outlook Form - help!

Hi all. I nned some help with a customised form in
Outlook and I'm hoping that one of you will have the
solution.

I have customised a form for use as an email questionnaire
and have included some tickboxes. My problem is, that
when people complete and reply to me, the tickboxes won't
stay ticked.

I have disabled the 'reply' option on the email form to
force people to use the 'forward' option. This ensures
that data input is preserved. But it doesn't seem to
affect these tickboxes.

I've had a look at the properties for the boxes but
nothing is jumping out as being the obvious cause. What
am I missing?

Ah that's done the trick! I used the control toolbox for
the initial tickboxes, everything else was created from
the 'field chooser'. I've now got five lovely tickboxes
that stay ticked.
