Custom notifications

Periodically a custom notification is gone, and I go re-set it to
"always show". It's as though "always show" has a time limit of a
couple weeks to a month. Is this a common situation?
 
They can be deleted by some utility programs. One such is MRU-Blaster, if
you have it set that way.

The 'Customize Notifications Past Items' are found in Windows XP. They
record every item that has placed an icon in your system tray.

It is important to note that clearing these items will also remove any
customizations you have made to the default Windows XP system-tray-icon
display settings. For example, if you have set the icon for MRU-Blaster
Scheduler to remain hidden in the system tray, clearing these past items
will also remove that setting.
