Custom Form

I have absolutely no experience in this area at all so I have got no idea if
this is possible or not.

I need to gather information from a list of users. I was hoping to create a
custom form with a number of tick boxes and fields on it. I would then
email this to users and they would open their email and see the form that I
have created. They should then be able to complete the fields on the form
and email back to me.

Is this something that can be done?

Yes, this is something that can be done *if* all the users are using Outlook for mail. See http://www.outlookcode.com/d/forms.htm to get started. The details will depend in part on what mail server you use.

FYI, there is a newsgroup specifically for Outlook forms issues "down the hall" at microsoft.public.outlook.program_forms or, via web interface, at http://www.microsoft.com/office/com...spx?dg=microsoft.public.outlook.program_forms

Yes, that is possible. A webbased poll or a mail based poll would also work.
