Creating Password for forms

I created a document usting the Form toolbar in word, I want to be able to
apply a password to the "protect form lock", so when people open the form
they can enter information only into the check and combo boxes but not
delete or edit other info on the form, but want to make sure they cant press
the lock button to unlock the form.

I have tried this but it when I go Tools => Protect Document it goes into a
three step dialogue box which goes 1. Formatting restrictions (ticked box
that says: Limit formatting to a selection of styles) 2. Editing restrictions
3. Start enforcement.

After I accept step 3. i.e "Yes, Start Enforcing Protection". It protects
the whole sheet but you can still unlock the form protection button on the
form tool bar but I want to protect this also.
 
When I follow these steps, selecting "Filling in forms" for #2, then when I
click "Yes, Start Enforcing Protection," I get a dialog asking for a
password.
