Creating Lables from excel spreadsheet in MS Word

I followed instructions and imported my worksheet data (3 addresses) into
labels, however, one label is blank & remaining labels have "Next Record".

Spent 2 hours going over each step with same results, any suggests where I
go next with this to merge my worksheet data to labels?
