Creating Forms in Outlook

I have some forms that are created and I and trying to put them into outlook.
It looks like it works but when you go into Outlook Forms it is not there.
I want to have this form in Outlook so that my users can pull it up and send
it to the mailroom.
 
I am using Outlook 2002 and yes I have an Exchange Server. I am trying to
publish a form in outlook for other users to use. These are the steps that I
am taking:\

Tools
Forms
Publish Form
Look in personal forms library
Select the form

Then when I go to forms in outlook the form does not appear.

Please tell us more about what you mean by "when I go to forms in outlook."
The commands to launch a form would be Tools | Forms | Choose Form.

Note that a message form published to Personal Forms can only be used by
*you*. If you want other people to use it, it will need to be in
Organizational Forms or in other users' Personal Forms.

We created a folder under our public folders called Forms and we wanted to
publish this specific form to that file.

This is a message form? It is not appropriate to publish a message form to a
folder. In order for it to work properly, in most instances, it needs to be
published to the Organizational Forms library.

You can then use a separate launcher form in a public folder to make it easy
for users to launch the message form. See

Where do you find "Organizational forms library?" When I go to publish my
custom form, I don't see that as an option.
 
The Organizational Forms library is a central library stored as an Exchange
Server system folder. The Exchange administrator must create it and grant
