Creating a questionaire - checkbox problems

Hi I have created a questionaire with check boxes and text boxes I have
protected the document to filling in forms only, this works perfect on my
computer however when I send it to my colleague for checking it
automatically starts in design view therefor he is unable to check any of the
boxes.

Does anyone know of a way to change this?

The presence of design view indicates that you used one or more
objects from the Control Toolbox instead of those from the Forms
toolbar. The recipient has the macro security level (Tools > Macro >
Security) set to High, so that macros and Control Toolbox objects are
disabled.

The correction is to remove any and all Control Toolbox objects and
replace them with form fields.
