Creating a form

I am trying to create a new form with not much luck.
I was wondering if anybody could help me?

I work at a front reception desk for a university and
I'm looking to create one that I can fill in while I've
got the person on the phone. (to pass the message on) like
a "while you were out" one but adding in more details
like "student Number" "program studied" "disciplin"

The hardest thing I have found is being able to get this
information inserted into the message text when you
receive it.

Is there any way? Help?
 
The most helpful thing I found is a Microsoft book titled
Building Applications With Outlook 2002.

If you look through these posts you'll see that I have
asked several questions just like yours. I was ready to
give up or hire a consultant. I sat down with this book
for about an hour and created 4 working forms.

There are a lot of things that I still need to look up
and probably a lot more that I do incorrectly but I
created 4 working forms and I'll just continue to get
better.

Buy the book. Even though it's from Microsoft it provides
useful information. Good luck.
 
What do you mean by "a combination list type"?
