Creating a Form to link with Access

Our company currently is using a linux box within our
intranet to submit a trouble ticket when a user is having
problems with the network and/or their computer. This
when submited sends a blank email to 4 email addresses of
our IT department. Then basically the most qualified
person takes that issue. These issues are then placed
into a public folder where we can read them. once they are
done we place them into a resolved issues folder. We
would like to make this a little more user friendly. Is
there anyway to create a form within outlook where a user
could type their problem, along with some other
information as well as make a priority such as low to high
priority? Then when submited could this form send a
notification to the 4 IT personel, then export itself into
an access database? We are just brainstorming Ideas for
future use. If anyone has information on how to program
something like this or know of a better way to accomplish
the same type of result I would certainly appreciate any
help at all. Thanks in advance!
