Checksheet

We have several forms that we use to track Audits that are done daily. I
have been looking for an example of a simple Checksheet in Access so that I
can lrean how to recreate those excel checksheets into Access Data that we
can track our monthly performance on each item.
Item 1 might be "Check for uncontrolled sheets in area." Y or N
Item 2 might be "Sheets are dry leaving the washer." Y or N

I then need to be able to make reports based on the results for a chosen
period of time.

This seems simple but I cannot seem to find a sample database with these
functions.

In general, you could have tables that have yes/no fields for your answers.
Create reports based on the yes/no field being true or not.

If you would like more specific help then please provide more specific info.

Thank you for your response, to give a little more detail I have not created
an Access Database before so I was hoping to find an example or templete to
learn from.
I believe that most companies have check sheets that are use to audit
different functions to see if tasks were completed or not. For example there
might be a from for the cleaning company to make sure that they cleaned the
carpets, tile, windows, bathrooms...... the check sheet might be filled out
once a week and then logged into the database. Then you could tally how many
times they missed the windows or the percentage of times they cleaned the
carpets. Another example would be a manufacturing plant that had certian
quality and operational check that an operator should do each shift. The
supervisor would come by with a check sheet to see if all the tasks were
preformed. That dat would be entered into the database and then reports
created.
So what I was hoping to find or recieve was an example of a database that
would do that type of data tracking and would have a form that could be used
to enter the data with checkboxes so a check would mean completed and empty
would mean not completed. The questions would remain the same but the person
doing the check would change and the date would change and even the shift
would change.

for "microsoft access templates" - you will find plenty of links.

I will continue to search but I had not had much luck finding a database
example with a checksheet form in it to update the database records. If you
know of one that would be helpful if not I will keep looking.
