Checkboxes on Word form disappear when sent through Outlook

I created a form in Word 2003 and sent it through Outlook. When I open the
e-mail all the checkboxes are gone. How do I get them to show up?
 
You mean you created checkboxes in a Word document and then sent that with the File | Send command in Word? What's your aim here?

Yes, File --> Send to. I created the form so users can fill it out and then
send to a Distribution List as an e-mail, not attachment.
 
Outlook doesn't support building survey type forms with Word fields. You might want to consider sending it as an attachment or, if you're in an Exchange organization, building a custom Outlook form instead.
