CheckBox issue

HI.
I have created form where form fields are filled throug UserForm dialog.
everything works well except CheckBox.
I add chceck box to form and also to UserForm but do not know how to conect
them together ( you know when user tick chceckbox in UserForm dialog in
form should be ticked check box too.

could somebody help me with macro please?

Thank you for your answer however I spent all the evening trying to make it
work but in spite of I checked all possible variations of labeling CheckBoxes
in UserForm and in form only thing I manage my PC force was dialog box saying
that UserForm11 does not exist but I am sure it does exist becaue I saw it in
VBA form list.
I think I throw in the towel.
